Madison Medical Back In-Network with UHC
We’re happy to share that Madison Medical Affiliates is once again in-network with all UnitedHealthcare insurance plans, effective October 1, 2025.
Ascension Wisconsin and UnitedHealthcare have finalized a new agreement that ensures continued access to in-network care for all UHC Commercial, Medicare, and Medicaid members. Any services received between October 1–13 will also be processed at in-network rates.
We are deeply grateful for your patience and understanding during this transition — and most of all, for the trust you place in us. It’s a privilege to care for you and your family, and we’re thrilled to welcome our UnitedHealthcare patients back with open arms.
As we work through this transition, please be patient with our team — we expect higher-than-usual call volumes over the next few days and will do our best to respond as quickly as possible.
Lisa Bennett, M.D.
Dr. Lisa Bennett is a board-certified Ophthalmologist who graduated summa cum laude at the University of Minnesota and earned her medical degree with honors at Chicago Medical School at Rosalind Franklin University. She completed an internship in Internal Medicine at the University of Chicago, NorthShore Hospital and a residency in Ophthalmology at Saint Louis University. Dr. Bennett is experienced in cataract surgery, astigmatism correction and refractive surgery, including blade-free LASIK and PRK. She also manages patients with glaucoma, macular degeneration and dry eyes. Dr. Bennett is on staff at Columbia St. Mary’s Ozaukee Hospital where she performs minimally-invasive cataract surgery and laser treatments for glaucoma, diabetes and secondary cataracts. She is committed to providing comprehensive eye care to ensure optimal patient outcomes.
