Self-Pay Policy, For Those Without Health Insurance

Madison Medical Affiliates and Ascension Columbia St. Mary's offer a 33% discount to any patient without health insurance. This discount does not apply to deductibles, co-pay/co-insurance or standing accounts not paid in full. To make a payment arrangement, contact one of our customer representatives at the number listed below.

For patients requesting financial assistance, Ascension Columbia St. Mary's has trained financial counselors available at each hospital. Upon making an appointment, the financial counselor will meet with you to review what other financial assistance programs you may qualify for. For more information, click here.

Refund Policy

Refund requests should be submitted to Madison Medical Affiliates' Customer Service department.

If no money is owed on other accounts, refunds will be issued within 90 day of receipt of your request. Refunds will be issued through a check delivered via US Mail. If money is owed on other accounts, the refund will be credited towards the oldest outstanding balance.
